Dean Certifications

Students and alumni who are applying to other academic institutions or for employment which requires a disclosure of prior conduct information are required to complete a Dean Certification or Student Conduct Verification. Because conduct records are considered educational records protected by FERPA, Student Conduct & Conflict Resolution does not share information regarding conduct records with anyone outside of FIU without a written consent. Thus, a release authorization from the student is mandatory.

Student Conduct & Conflict Resolution does not release prior conduct information over the phone as we are unable to verify the identity of the caller. If you are unsure of your prior conduct history, please submit a Student Conduct Records Request Form. Obtain the form by contacting the Student Conduct office.

If your form requires graduation verification, transcript request, or any type of academic verification, please send your form directly to the Registrar's Office and they will forward it to us once they have completed that portion of the form. Our office will then complete the form and submit it to the requested institution.

Process and Timeline

Typical processing time is 10-15 business days. Below are the steps in the process.

  1. Student submits applicable forms that need completion
  2. SCCR forwards forms to Registrar for completion (if applicable)
  3. Registrar completes and returns to SCCR
  4. SCCR processes requests on Fridays only
  5. SCCR sends from to respective institution(s)

What You Need to Know

Common Application – Transfer Registrar's Report

FIU will only complete the HARD COPY, not the online version. You will need to complete a Student Conduct Record Requests Form correctly and completely. Please make sure you provide a valid email address and telephone number that is active.

How to get the "hard copy" of the online Common App

The online Common App still requires that a student submit a Registrar name to complete their application. Follow these instructions to access the paper form and still submit your application online:

Go to "Assign Recommenders"→Manage Recommenders→add "Kevin Coughlin" as the Registrar, leave the email address blank→Choose Offline Form→Print-Out →Submit form to GC 311 (Student Conduct & Conflict Resolution office) with Student Conduct Records Request Form.

You do not have to take your Common App to multiple offices

Once you have completed the Student Conduct Records Request and attached your copy of the Transfer Registrar's Report, the office staff will forward it to the Registrar's office to complete their section and return to our office for completion.

All Other Types of Transfer/Admission/Study Abroad/Reference Forms

FIU will fill out the hard copy reference forms only. If you need a transcript as well as a Student Conduct Records check, please visit the Registrar to obtain the transcript prior to bringing your forms to the Student Conduct & Conflict Resolution office.

Take any document requiring a character evaluation to a Professor, T.A (Teacher Assistant) or Dean of your College and ask them to fill out the evaluation portion of the form. Please make sure they do not sign the form and are not filling out your dates of attendance and if you are in good standing with the University. The school Registrar and/or Student Conduct & Conflict Resolution staff are the only school officials that can validate this information.

Please ensure that any form with sections requiring your signature to release records have been signed and you have filled out any sections designated for the student to complete. Student Conduct & Conflict Resolution staff will not release any documents with any student signature field that has been left blank.

Requesting A Letter of Good Conduct Standing

Student Conduct & Conflict Resolution will provide a general notice to verify good standing or informing the recipient of the general facts of any incident, violations, and sanctions as applicable.

How to Have Your Records Sent

Fill out the Student Conduct Records Request Form. No records requests will be accepted without both the top and bottom sections completed with the appropriate information (i.e. fax numbers, email address, stamped envelopes, etc.)

Fax: Students must provide the name of the Institution/School/Agency as well as the fax number including area code.

We can fax the same form to multiple locations. You do not need to submit multiple Student Records Request Forms. Please write any additional Institution/School/Agency names and fax numbers, including area codes, on the back of the Records Request Form.

Email: Student must provide the name of the recipient at the Institution/School/Agency and their email address.

Pick-up from Student Conduct & Conflict Resolution office: Students must include a phone number they will answer as well as a check their email for correspondence regarding their records request.

US Mail: Students need to provide a stamped addressed envelope for each recipient they want a paper copy mailed to. Use address and return address format as indicated below.
