We have created forms for our most popular requests to enhance engagement with the FIU community as well as to streamline our tracking process. Please choose the one that better fits your needs from the list below:

  • Records Request for CAPS/VEP

    Requesting Records

    Complete the online form and submit; a representative from CAPS/VEP will attempt to contact you within 3-5 business days to determine the nature of your request and. A Release of Information and/or Records Request Forms may be provided for you to complete.
    • Forms must be signed and submitted via FIU e-mail to the Counseling Center as an original document if you are unable to complete the form in person. If you are no longer at FIU, you still have access to your FIU email. If you have questions, please click here.
    • After you complete the required forms, CAPS/VEP will respond to your written request within 30 business days. You will be notified when the records are ready.
    • Responses to requests for information contained in a confidential record will be delivered in person, via fax, via USPS Certified Mail, or to FIU emails.
    • Bring a photo ID to pick up records in person.

    Please note: you may also speak with your regular clinician about records requests during a scheduled session

    In addition:

    CAPS does NOT provide forensic psychological assessments and related documentation for:

    • Emotional Support Animals
    • Housing contracts/living alone
    • Workplace disability
    • Court mandated treatment or evaluations

    FIU CAPS/VEP Records

    CAPS/VEP maintains confidential records in accordance with state law. Records are maintained for 7 years past the last date on which the last service was provided. Information regarding contact with our center will not be disclosed to anyone outside of CAPS/VEP without the client’s written authorization to release the information except as mandated or allowed under Florida and/or Federal Law.

    In situations where an exception to confidentiality protection may exist, CAPS/VEP will make an effort to discuss the circumstances and procedures for the disclosure with the client and enlist the client's assistance in resolution of the situation.

    The CAPS/VEP clinicians offer a series of workshops and events that are scheduled at no cost to the FIU Community, upon request.
