Insurance FAQ

Please be advised that during this period of remote learning due to the coronavirus outbreak all international students must submit proof of insurance coverage before registering for classes for the Summer/Fall 2020 semesters. Students taking classes online within the United States during this time must submit proof of insurance to remain compliant with the regulations set forth by the Florida Board of Governors (6.009) for F1 students. Students who have left the country must contact ISSS (305) 348-2421 to change their Visa status.

  • I am an International Student and I have an insurance hold on my account. What do I do?

    According to Florida Board of Governors and FIU policy, all International Students with F1 and J1 visa status are required to have medical insurance prior to registration. Proof must be submitted to the Health Compliance Office at the Student Academic Success Center (SASC Lobby or insurance@fiu.edu).

    Students will not be allowed to register for classes or continue enrollment without complying with the medical insurance requirement. There are no exceptions to this rule.

    The medical insurance requirement can be met in one of two ways:

  • Are all students required to submit insurance?
    Only international students are required to have insurance, as per Florida state regulations. Medical and Nursing students may also need to have insurance to satisfy their program requirements.
  • How do I purchase the FIU Insurance Plan?

    FIU partners with the State University System’s Insurance Broker, Gallagher Student Health & Special Risk, to provide international student health insurance coverage through United Healthcare Insurance Company.

    To be eligible to purchase the plan, students must be full-time, with at least 50% of the credits being taken on campus. Students can enroll in the insurance online by visiting the website www.gallagherstudent.com/fiu.

    International students should click on the link “International Student Enroll” to make an account and continue the enrollment process. There will be a nominal processing fee for transactions. Once payment is processed, the student will receive a Confirmation of Payment email from the Gallagher website. The student must forward the Confirmation of Payment to insurance@fiu.edu to remove their insurance hold.

  • How much does the FIU insurance cost?

    The price for the FIU Sponsored Insurance Plan varies from semester to semester. Students can find the most current pricing for the insurance plan on the Gallagher website, www.gallagherstudent.com/fiu or on the FIU Student Health website, studenthealth.fiu.edu, under “Insurance/Insurance for International Students”. You can also contact Gallagher Customer service at 1-877-498-5468.

  • What does the FIU insurance cover?
    For more information concerning coverage benefits of the FIU Sponsored Insurance, please visit www.gallagherstudent.com/FIU or visit the FIU Student Health Website: studenthealth.fiu.edu. Click on “insurance” then, click on “insurance for international students”. Scroll until you see “FIU Sponsored Health Insurance Plan” and you’ll see more information about Gallagher and also FAQ provided by Gallagher. Remember, you can also contact Gallagher Customer service at 1-877-498-5468.
  • I have the FIU insurance and I believe that I was incorrectly charged for a service. What do I do?

    You can contact United Healthcare at 1-866-948-8472 or send an email to gkclaims@uhcsr.com to make a claim for reimbursement.

    If the service you received was at the FIU student clinic and you used your Gallagher insurance and believe you were incorrectly charged, check in with the clinic first at 305-348-8385 and after you can contact Gallagher or follow the instructions given by the clinic.

  • Can I purchase an annual plan for the FIU insurance?
    The FIU Insurance Annual Plan is available for purchase in the Fall term only. The coverage period for the annual plan usually starts on August 17th and ends on August 16th of the following year. Monthly payment option is available for Annual plan purchases only.
  • I just purchased the FIU Insurance. How can I access my ID card?
    ID cards are available on the Gallagher website 5-7 business days after the insurance is purchased. You can access your card by following these steps:

     Go to uhcsr.com/MyAccount

    1. If you have created a UHCSR Account in the past, log in
    2. If you have not created a UHCSR Account, Click “Create Account”
    3. Complete the registration form using your name, birth date, and student ID number
    4. Once logged in, you will have access to ID cards, Claim information, EOBs, and other plan-related information
  • How do I find and submit the Alternative Insurance Compliance Form?

    The Alternative Insurance Compliance Form can be found on the FIU Student health website, studenthealth.fiu.edu under the “Forms” tab then clicking on “Insurance forms”.

    The Alternative Insurance form will be approved only if it abides by the Florida Board of Governors state regulations listed on the Alternative Insurance Compliance Form. Students may not submit their insurance card as proof of alternative insurance.

  • I have an alternative insurance. What are the valid dates of coverage that are acceptable for the requirements?

    All alternative health insurance companies must adhere to the minimum dates of coverage listed at the bottom of the first page of the FIU Alternative Insurance Compliance Form, as shown below. These dates are consistent with the coverage set forth by the Florida Board of Governors for international student health insurance requirements.

  • My insurance is covered by my Graduate Assistantship/Fellowship. How do I remove my insurance hold?
    Graduate Assistants and Fellows must contact the University Graduate School at 305-348-2455 to verify their insurance enrollment. Insurance enrollment must be verified every semester for international students who are Graduate Assistants/Fellows.
  • What if my insurance is covered by my government?

    If your insurance is covered by a foreign government, you need to contact the Student Health Advisor(s) of your embassy and request that they complete the Alternative Insurance Compliance Form. Once completed, it needs to be sent to us via email directly from the representative who completed the form.

    All compliance forms must have a specific start date and end date in accordance with the minimum dates of coverage listed at the bottom of the first page of the FIU Alternative Insurance Compliance Form. These dates are consistent with the coverage set forth by the Florida Board of Governors for international student health insurance requirements.

    We will not accept Financial Guarantee Letters/Letters of Financial Support under any circumstances. There are no exceptions.

  • What if my insurance is covered by a Fulbright scholarship?
    Please send your Fulbright award letter to insurance@fiu.edu to remove your insurance hold. Your award information will be sent to the insurance company for billing. Please contact your Fulbright sponsor for updates concerning insurance billing and enrollment.
  • What if I am international student, but my visa status was recently changed?

    The insurance requirement is mandatory for all students with an F or J visa status. If your visa status has recently changed, please contact the International Student Services Office at 305-348-2421 to update your account. Your insurance hold will be removed once your account is updated.

  • What if I am not an international student, but I wish to purchase insurance with FIU?

    Full-time students with on-campus courses are eligible to apply for the FIU Insurance Plan. Please visit www.gallagherstudent.com/fiu to enroll.

  • I am graduating during the Spring semester. What are the valid dates of insurance coverage I need to remove my hold?

    All students taking classes during spring semesters must have insurance coverage for both the Spring and Summer semesters (even if you’re graduating in the Spring term or mid Summer term) this is in accordance with the coverage dates listed on the FIU Alternative Insurance Compliance Form which is in compliance with the regulations set forth by the Florida Board of Governors. The coverage dates for Spring 2021 and Summer 2021 semesters are from at least January 1st 2021 until at least August 16th.

    There are no exceptions.

  • My insurance coverage is provided through my job/I am a dependent student covered with my spouse’s insurance which is provided through their job. Is this acceptable to meet the university’s insurance requirement?

    The alternative insurance will be approved only if it abides by the Florida Board of Governors state regulations listed on the Alternative Insurance Compliance Form. Students may not submit their insurance card as proof of alternative insurance. A Human Resources representative from the company of the insured, or an insurance agent must complete the Alternative Insurance Compliance Form before we can confirm if the plan will be accepted.

    All alternative health insurance companies must adhere to the minimum dates of coverage listed at the bottom of the first page of the FIU Alternative Insurance Compliance Form. These dates are consistent with the coverage set forth by the Florida Board of Governors for international student health insurance requirements. If no end date can be written as per the HR representative, an additional document written on the company’s letterhead must be sent along with the Compliance Form verifying that the company will notify FIU Health Compliance should the insured’s employment terminate prior to the end of the semester.

    Please note that Compliance Forms lacking a specific end date can only be accepted for one semester at a time. Students must submit a new compliance form and supporting documentation for every semester if no end date is provided on the form.

    The completed Compliance Form and supporting documentation must be sent directly to the Health Compliance office via email to insure@fiu.edu for review and processing.

  • What if my insurance policy doesn’t cover Repatriation and Evacuation?
    The FIU Sponsored Insurance, Gallagher, offers a plan just for Repatriation and Evacuation for $60. Please visit their website gallagherstudent.com/FIU log in or create an account in order to purchase this plan. If you need to help to purchase the repatriation and evacuation plan, contact Gallagher Customer Service at 1-877-498-5468.
  • I’m a Global First Year international student, what do I do?

    Your G1Y advisor/coordinator, will provide you with the option to purchase insurance from G1Y. If you decide to purchase insurance with G1Y, they will complete the FIU Alternative Insurance Compliance Form for you and it will be valid for one year. However, PLEASE KEEP IN MIND, that this is only for your first year at FIU. After this insurance coverage ends, you are responsible of searching for an insurance company that best fits your needs and ask them to complete the FIU Alternative Insurance Compliance Form. Your other option is to purchase insurance from Gallagher which is FIU’s sponsored insurance company. If you choose to purchase Gallagher insurance, you are only responsible for sending us the confirmation of payment email you will receive from them. Our email is insure@fiu.edu .
