Vaccine Incentive Program

  • What is the vaccination incentive program?

    Beginning August 25th, unvaccinated students, faculty, and staff are eligible to receive a $150 FIU One Card incentive payment if they get vaccinated.

    • Student enrolled for Fall 2021 or active employee
    • Obtains first vaccine dose of Pfizer or Moderna vaccine between August 25 and September 30, or
    • Completes their vaccine series (for those who only had one of two doses prior to Aug. 25) by October 31
    • Those who receive the single dose Johnson & Johnson vaccine must receive it by September 30.
  • Who is eligible for the incentive program?

  • Do I need to get vaccinated at FIU to be eligible?

    Although the incentive process will be faster if you’ve been vaccinated on campus, NO, you do not need to get vaccinated at an FIU site. All students, faculty & staff that become fully vaccinated (i.e., completed their series) between August 25 - October 31, 2021 are eligible. There are also multiple locations locally where you can get vaccinated.

  • Where do I upload my complete vaccination card?
    1. Login to the Student Health portal (this is for students, faculty and staff) using your FIU credentials
    2. Select the Document Upload Tab
    3. Click- “Document Type” and select COVID-19 Vaccine Card
    4. Browse for the file you will upload and once selected click “Save

    By uploading your documentation, you agree to grant permission to the FIU Student Health Center to access your Florida SHOTS record. Please call 305-348-6658 if you are unable to log in to the portal. Assistance is available, Monday through Friday, between 9 AM - 5 PM.  

  • I logged in but cannot access the Student Health Portal to upload my vaccination card. What can I do?

    Please call 305-348-6658 if you are unable to log in to the portal.  Assistance is available, Monday through Friday, between 9 AM - 5 PM. You can also visit https://go.fiu.edu/p5form to request access. Please allow 1-2 business days for processing.

  • Are there tax implications if I accept this incentive?

    For students

    • US Citizens and resident aliens – This vaccine incentive payment is considered taxable income and may be reported to the Internal Revenue Service (IRS) on form 1099-MISC if taxable payments to the recipient exceed $600 during the calendar year.  If total payments made during the calendar year are less than $600, FIU will not prepare a 1099-MISC form, but the payments are still considered taxable income and should be self-reported by the recipient.
    • Non-resident aliens (NRA’s) – This vaccine incentive payment is considered taxable income, is subject to Internal Revenue Service (IRS) reporting on form 1042S, and is subject to a mandatory 30% withholding tax.  In compliance with federal tax law, FIU must withhold 30% tax from the payment resulting in a net payment to the NRA recipient in the amount of $105. If an NRA recipient is new to FIU and has not been taxed in the past, the recipient must contact the Office of the Controller, Tax Compliance and Payroll Accounting Services via email tax@fiu.edu for creation of a tax record, submission of required forms, and tax status determination. The vaccine incentive payment cannot be issued until the tax record is current.

     

    For employees

    • The vaccine incentive payment is considered taxable income, included in gross income, and subject to tax withholding. Federal withholding, Social Security and Medicare taxes will be assessed on the value of the benefit on the corresponding pay period.
  • Do booster shots count towards the incentive program?
    No. Only the main vaccine series (i.e., 1 Johnson & Johnson or 2 for Moderna/Pfizer) will qualify.
  • What if I am already fully vaccinated? Am I eligible for the incentive?
    For those that were fully vaccinated before August 25, you are eligible for the Vaccine Reward Program. Please refer to the next section of FAQ.

Vaccine Reward Program

  • What is the vaccination reward program?
    Anyone that was fully vaccinated before August 25th will be entered into a raffle where 300 participants will be selected to receive a $150 incentive. Winners will be notified within a month of uploading a valid vaccine card.
  • Who is eligible for the reward program?
    • Be an enrolled student or active employee during Fall 2021
    • Fully vaccinated before August 25
    • Upload your vaccination card by September 24th
  • Where do I upload my complete vaccination card?
